Open New Account


MFS Funds are available only through an investment professional; we cannot accept applications without a registered representative listed. We are confident an investment professional can provide you significant value by working with you to identify investments to best fit your goals and objectives.

To open a new account with MFS, please follow the steps outlined below. For additional information, be sure to read these tips for setting up your account.

Step 1: Complete the new account application with your investment professional

The new account application can be downloaded, printed, and completed by hand or you can fill in the application online and print it out. After filling out the application, each registered owner must sign it and your investment professional's firm must complete the dealer section.

Important: To help the government fight the funding of terrorism and money-laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. Please work with your investment professional to ensure the required information is provided on the application.

MFS cannot establish your account unless Sections 1 through 5 of the application are completed. (Download application below.) We also suggest reviewing other account services offered in Sections 6 through 10 with your investment professional. Note: Do not use the MFS Mutual Fund Application to set up an MFS IRA or 529 College Savings Plan.

MFS Traditional and Roth IRA Forms

  • IRA Forms Kit
    Provides all forms necessary to open, convert, directly roll over or transfer a traditional or Roth IRA at MFS. Complete this form on your computer, print it, and mail it to us. Investors need to obtain an authorized signature from their dealer firm prior to mailing this application to MFS.
  • IRA Transfer Form
    Transfer funds into an existing MFS IRA (if new account needs to be established, see IRA Forms Kit, under Open a new account). Complete this form on your computer, print it, and mail it to us.
  • Roth IRA Conversion Form
    Convert assets from an MFS or non-MFS Traditional IRA to an MFS ROTH IRA. Complete this form on your computer, print it, and mail it to us.
  • Disclosure Statements and Trust Agreements
    IRA Disclosure Statements and Trust Agreements to accompany the IRA Forms Kit.
  • Retirement Plan Service Form
    IRA Service Application includes reduced sales charges, dealer changes, dollar-cost averaging, invest by phone, bank account information and automatic investment.
  • Investment Instructions to Establish a New IRA Account(s) in a Different Class of Shares
    Completing this form will create a new account within your existing MFS IRA Trust invested in a share class that differs from what was elected on your original IRA Application. This form cannot be used to move funds between share classes.

Tips for setting up your account:
  • Make sure to register the names on the account the same way you register other financial accounts. For example, if you sign your name and have other accounts registered "John R. Jones," make sure to use the same format on your MFS account (not John Jones or J.R. Jones).
  • Fill in the application completely and print clearly. Leaving out information may delay getting your money invested or we may have to send the application back.
  • Link your checking/savings account information so you can quickly and easily add money to or withdraw money from your mutual fund account.
  • Individuals: Consider completing a "Transfer on Death" (TOD) registration request to quickly transfer proceeds to your named beneficiaries in the case of death.
  • Trusts, corporations, partnerships, limited liability companies (LLC), and other organizations:
    -Provide complete documentation including, as applicable; the Articles of Organization/Incorporation, Trust Agreement, or partnership agreement.
    -Provide documents identifying person(s) with authority to over the account.
    -List the names of Trustees or authorized signatories on the application with the appropriate tax identification number (TIN).
